Age Friendly Health Care is an approach to health care developed by the Institute for Healthcare Improvement. It focuses on 4 areas of care: What Matters, Medications, Mentation and Mobility. This highly interactive two-part presentation will focus on what matters as a way of communicating about your healthcare with providers, family and friends.
Handouts will include practical tools that have been developed and tested by geriatricians.
The presenter, Dr. Kathyrn Borgenicht, is a member of Northwest Village Network and is a retired geriatrician and palliative medicine physician who has worked with older adults for many years. She has focused on keeping people functional and helping them communicate their needs to their families and medical providers.
